The consent form is essential for ensuring patient and staff safety, strengthening communication with the patient, increasing patient engagement, sharing responsibilities with the patient, and addressing the legal aspects of patient and staff safety in accordance with legal processes. It also ensures that the patient is informed and their consent is obtained.

Through Medicasimple, you can easily send a consent approval SMS to your patients or print the form for them to sign manually.

To configure the consent form, navigate to Settings > Forms - Consent Form.

If no form has been set up previously, a blank screen will appear. To create a new consent form, click the New Consent Form button on the right.

Clicking the button will open a new window where you first need to set a form title for the consent form.

After naming the form, you can enter its content in the text field below. If you do not have a pre-prepared consent form, you can access the TDB Consent Form from this section.

If you want the treatment plan to be displayed in the consent form, check the "Show Treatment Plan in Form" option at the bottom.

Finally, click the Create button to generate your consent form.

Now that your consent form is created, a separate consent form will be generated for each treatment plan you create.
